REGISTERING AS A PATIENT
Patients wishing to register must live within the Practice Boundary. We will not refuse to accept a patient on the grounds of race, gender, social class, age, religion, sexual orientation, disability or medical condition.
Please bring in your signed Medical Card (or Passport/Driving Licence identification) to complete the Practice Registration form, together with a Health Questionnaire. It is also helpful if you bring in details of your current medication, if any, your immunisation record if known and information on any hospital treatment you are receiving, if appropriate.
An appointment with a Practice Nurse is made for a New Patient Health Check and we then apply for your full medical records from the Health Authority. It is essential that new patients attend their New Patient Health Check appointment to enable us to evaluate your medical requirements, screen for common disorders and discuss your medication and any concerns you may have.

Temporary Patients
We will be pleased to see anyone staying with, or visiting you, if they require a doctor or a nurse, and cannot attend their own surgery, due to distance.

FLU CLINICS
Flu vaccinations begin on 1st October 2011 and "Flu Clinics days, including some Saturdays will be held at both Surgeries.
Please check with Reception for appointment availability, and eligibility for the immunisation. The Practice cannot offer "Private" flu vaccinations to patients.
Training Practice
The Practice is a GP training Practice, for information, our Registrars are fully qualified Doctors who have decided to specialise in General Practice.
EXTENDED OPENING HOURS
The Surgery at Billingborough offers appointments between 6.30pm and 8.00pm, these appointments are in place for our working Patients who are unable to attended during normal surgery times. As these appointments are limited please use them responsibly.
DIY SICK NOTES
It is estimated that more than 2million NHS appointments and 40,000 hours of GP’s time are wasted each year by people requesting a Doctor’s Sick (now “Fit”) Note when they don’t need one. If you are away from work for less than a week you can self-certify using forms SC1 or SC2 which are available from your employer.

We continue to have problems with patients who DNA (do not attend) a booked appointment. Today, for instance, 12x 10minute appointments have been wasted = 2 hours of Clinician’s Time.. PLEASE ring to cancel any appointment you cannot attend so that another patient can be seen.
General Information
Statement of Fitness for Work (effective 6th April, 2010) - Information for patients is available online at www.direct.gov.uk
You do NOT require a Certificate for any illness lasting 7 days or less. Your employer may require you to complete a Self-Certification Form, which he will give to you.
NHS General Practitioners are required to issue, free of charge, a Statement of Fitness to Work for patients for whom they provide clinical care. Under the new "Fitness for Work" system, your Doctor may advise that your medical condition does not necessarily prevent you from returning to work - though arrangements regarding normal duties or working hours may need to be amended.
Test Results
Test Results may be obtained by telephoning the surgeries between 11.00a.m. and 3.00p.m. Reception staff will pass on results, as directed by the Clinical Staff, and can confirm whether a follow-up appointment is required. To maintain Patient Confidentiality, results will only be given to the patient who has had the tests.

Please note that the Nurse's Treatment room is on the 1st Floor at Rippingale Surgery. If this causes you difficulty, please request appointments in Billingborough.
The Medical Equipment Fund
For over 20years the Practice has been supported by "The Springwells Medical Equipment Fund" which is organised and managed by a Patient Group. At the Billingborough Surgery, we receive and sell, good quality books, CDs and DVDs to raise funds. Specialist equipment, as agreed with the Patient Group, is purchased from the monies raised.
