New Patient Registration

Patients wishing to register must live within the Practice Area. To register please ask at reception, you will be required to bring photo ID i.e. passport & confirmation of address or driving licence, NHS Number and complete a registration form which is available from Reception or can be downloaded from the bottom of this page. It is helpful to provide details of any medication and hospital treatment you may be receiving.

We will not refuse to accept a patient on the grounds of race, gender, social class, age, religion, sexual orientation, disability or medical condition.

An appointment will be made for a new patient medical which enables us to evaluate your medical requirements, screen for common disorders and discuss medication and any concerns you may have. We then apply for your full medical records from the Health Authority.

Named Accountable GP for all Patients

• All current and new patients registered with this Practice will be allocated a named GP.
• If you would like to express a preference as to which GP you are assigned the Practice will make reasonable efforts to accommodate this request.
• Your accountable GP is responsible for your overall care at the Practice, however you are welcome to see any GP within the Practice.
• If you would like to know the name of your accountable GP, please ask at the Reception desk.

They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

We need to be informed of any changes to your personal details. This can be done by  contacting Reception and completing the relevant forms.

Once the forms are completed you will need to bring them back to the Surgery with Proof of ID and proof of new address (if changing) before the changes can be implemented.
